Devcon
Construction, Inc. requests that you submit a Certificate
of Insurance that fully complies with Item 9 and
Paragraph J of the General Conditions contained
in our subcontract agreement. The following should
be included on the certificate: |

Comprehensive
General Liability Insurance limits of not less than
$1,000,000. combined single limit for bodily injury
and property damage. |
|
Comprehensive
Automobile Liability Insurance limits of not less
than $1,000,000. combined single limit for bodily
injury and property damage. |
|
No cancellation
or reduction in coverage without thirty (30) days
written notice. |
|
Devcon Construction,
Inc. to be named as additional insured with the following
verbiage "Certificate Holder is named as additional
insured for all work performed by or on behalf of named
insured at all locations under contract with the certificate
holder." |
|
Subcontractor’s
insurance shall be primary and non-contributing
with respect to insurance carried by Devcon Construction,
Inc. |
|
Provide
coverage for Completed Operations Liability. |
|
Workman’s
Compensation limits of not less than $1,000,000.
per accident. |
|
Errors
& Omissions (Professional Liability) insurance
limits of not less than $1,000,000. |
|
An additional
insured endorsement for the general liability (commonly
referred to as a Form B or CG2010, etc.). This is
a separate page that should be attached to the certificate
of insurance. |
